Project Details
With a one-to-one case management model and working hand-in-hand with different professionals, we provide tailor-made and personalized support services for cancer patients, caregivers and survivors. Moreover, various activities (e.g. symptom management, life reconstruction and nutrition information) are ready to support cancer survivors. Besides, the Project is focused on the support for end-of-life patients, especially those in need of home care and palliative treatment services.
